Findings
The Alleged Victim (AV) has a known history of not being spatially aware of where his/her left arm is in regard to objects around the facility. The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs, relating to lack of care planning and interventions. On or about April 26, 2022, AV was unable to get through an exterior door before it closed on his/her left hand, which caused the door to pinch AV's finger, resulting in a fracture and requiring stitches. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
