Findings
Alleged Victim (AV) relies on the facility for his/her care. AV is a high fall risk and is care planned for Adhesive strips applied to shower floor for safety. On or about April 29, 2022, AV slipped in the shower causing a small abrasion. The facility failed to follow AVs care plan, which is a violation of resident’s rights is neglect of care and constitutes abuse.
