Findings
Alleged Victim (AV) relies on the facility for his/her care. Between February 2022 to May 3, 2022, AV has been found with skin tears, bruising and/or abrasions on his/her arms or legs on 13 occasions. On or about May 2, 2022, AV was found with a very deep skin tear to the head which required stiches. AV continued to have injuries and no new interventions were noted after every occurrence to prevent new injuries. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
