Findings
On or about April 12, 2022, the Alleged Victim (AV) was given the wrong dose of medications for multiple days. AV reported having a stomachache, was placed on alert for seizures, and was required to get additional blood work before resuming the medication. During the investigation there were additional medication errors noted. The facility failed to provide a safe medication administration system, which is violation of resident’s rights, is considered neglect of care and constitutes abuse.
