Findings
On or about April 20, 2022, the Alleged Victim (AV) suffered a fall in his/her room. AV stated he/she fell from his/her shower chair. AV was sent to the hospital and had suffered two fractured ribs and a laceration to his/her head. The facility failed to implement interventions to ensure AV's safety regarding falls. The facilities failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
