Findings
On or about May 9, 2022, it was discovered by staff that the Alleged Victim (AV) had ants crawling on his/her bed and his/herself. AV complained that the ants were biting him/her. The faciilty failed to control pests in the facility, placing AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
