Findings
The facility failed to properly care plan and implement reasonable interventions that would mitigate Alleged Victim (AV) from falling. AV fell twice while attempting transfer on or about May 07, 2022, and again on or about May 09/, 2022 sustaining a broken arm. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
