The facility failed to implement interventions and appropriately care plan related to AV’s history of falls. The failure resulted in AV experiencing multiple falls from approximately February 25, 2022, and May 14, 2022, which resulted in repeated unreasonable discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
