- On or about May 17, 2022, it was discovered that Alleged Perpetrator (AP2) put two (2) right shoes on Alleged Victim (AV). AV reported to AP2 the wrong shoe was uncomfortable on h/h foot, to which AP2 did not correct or check AV’s shoes for any issues. AV went to an appointment outside the facility with the right shoe on his/her left foot, causing unreasonable discomfort. AP2’s failure to properly dress AV is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility’s failure is a violation or Oregon Administrative rules.
