Findings
Alleged Victim (AV) has a known history of bruising and skin tears. AV has pads on his/her wheelchair to help prevent injury but will pull the pad off. AV has had two (2) previous injuries to his/her leg and there have been no new interventions added. The facility failed to care plan around AV removing the pad to the wheelchair, which is a violation of resident rights, is neglect of care and constitutes abuse.
