Findings
Alleged Victim (AV) has a history of bruises and skin tears. AV wears protective sleeves over his/her arms to prevent injuries but he/she is able to take off the protective sleeves and will not always be waring them. AV has continued to have injuries to his/her arms with the current interventions in place. The facility failed to care plan around AV removing the sleeves from his/her arms, which is a violation of resident rights, is neglect of care and constitutes abuse.
