Findings
Alleged Victim (AV) has a known history of lower leg bruising. On or about May 16, 2022, staff noticed two small discolorations to the front and back of AV’s right lower leg. No interventions were implemented after a prior incident from February 5, 2022. The facility failed to properly plan care, which is a violation of resident rights, is neglect of care and constitutes abuse.
