Findings
On or about May 19, 2022, was exhibiting increasing behaviors and eloped through a facility window. AV was found approximately four days later, dehydrated with sun exposure burns on his/her face and was taken to the hospital for treatment. The facility failed to follow the Alleged Victim’s (AV) care plan regarding progressive interventions for behaviors, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
