Findings
On or about May 20, 2022, the Alleged Victim (AV) suffered a fall where he/she hit his her head and complained of being dizzy. Facility staff did not notify the facility Nurse right away or send AV out to the emergency room after this fall to have him/her evaluated, even though facility policy is to notify the facility Nurse and send resident to the emergency room for evaluation, placing AV at risk for serious harm. The facility's failure is a violation of resident rights, is considered neglect of care an constitutes abuse.
