From approximately May 13, 2022, to May 18, 2022, the Alleged Victim (AV) exhibited increased behaviors, including confusion and irritability. The facility failed to intervene when AV experienced a change of condition, which resulted in AV experiencing unreasonable discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
