Findings
On or about May 24, 2022, Alleged Perpetrator #2 (AP2) gave medication to Witness #1 (W1) that belonged to the Alleged Victim (AV). AP2 knew the medication did not belong to AV and falsified documentation regarding the medication by marking it on AV's MAR as given to AV instead of W1. AP2's actions are a violation of resident rights, considered neglect of care and constitutes financial abuse. The facility failed to protect AV from financial abuse, which is a violation of Oregon Administrative Rules.
