Findings
On or about May 25, 2022, it was noted that the Alleged Victim (AV) had extensive bruising to his/her forearms. The cause of AV's bruising is unknown. The facility failed to implement interventions to reduce the number of bruises that AV has. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
