Findings
The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs, relating to transfers, bathing, toileting including bowel and bladder care, and wound care due to lack of staff, and as a result AV has experienced repeated unreasonable discomfort, risk of serious harm and a loss of personal dignity. These failures are a violation of resident rights, is considered neglect of care and constitutes abuse.
