Findings
On or about April 27, 2022, the Alleged Victim (AV) experienced a medical emergency and facility response was delayed. AV was ultimately sent to the hospital for treatment. The facility failed to provide a safe environment, which caused AV unreasonable discomfort, is a violation of resident rights, is neglect of care and constitutes abuse.
