Findings
The facility failed to follow the care plan. Alleged Victim (AV) is care planned to use a wheelchair for all ambulation, and a two-person transfer with a gait belt. On or about, June 07, 2022, AV experienced a fall when standing on a bus lift with h/h walker. AV reported left side ache and sustained a skin tear to left finger. This failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
