Findings
On or about June 8, 2022, the Alleged Victim (AV) had a bruise of unknown origin on his/her wrist. AV has a history of bruises at the facility. The facility failed to properly care plan for AV to avoid bruising. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
