Findings
On or about June 11, 2022, the facility failed to follow Alleged Victim's (AV) care plan, which stated to check on AV once a shift and notify nurse if AV misses a meal. It was unclear if staff checked on AV and the nurse was not notified after AV missed two (2) meals. AV was later found on the floor in their room by his/her family, with soiled pants. AV’s bed appeared to not have been slept in, AV appeared to be wearing clothes from the day before, and AVs medications were not taken all day. AV was transported to the hospital where he/she was admitted. Failure to follow the care plan resulted in causing AV a loss of personal dignity and risk of serious harm. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
