Findings
The Alleged Victim (AV) receives his/her medication from the facility. On multiple occasions, the facility has no record of AV receiving his/her medication. AV's primary care physician changed, however, the facility didn't update their records, resulting in AV being sent to the wrong physician's office more than once. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
