Findings
According to documentation, the facility failed to provide a safe environment to prevent injury. The failure resulted in the Alleged Victim (AV) sustaining an injury of unknown cause to their head, causing bruising. The facility failed to notify the RN of the injury and obtain a proper assessment for further injury, which is neglect of care and constitutes abuse.
