Findings
The facility failed to implement interventions and appropriately care plan related to AV’s history of falls. AV experienced approximately eighteen falls, with the majority being unwitnessed, from approximately May 26, 2022, and June 20, 2022. AV experienced repeated unreasonable discomfort, including skin tears and bruising. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
