- The Alleged Victim (AV) receives prescribed medication from the facility. On or about June 20, 2022, AV was given 100 units of a medication that controls blood sugar. AV was only to receive 12 units. AV was later found unresponsive and sent to the hospital for treatment. The facility's medication administration system was not clear on how much medication to give AV, resulting in staff giving 100 units rather than 12 units. The facility's failure to have a medication administration system that depicted clear instructions to staff placed AV at risk for serious harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
