Findings
The facility failed to properly care plan for the Alleged Victim's (AV) change of condition/behavior which resulted in the AV not receiving hygiene services for approximately two weeks which put the AV at risk of harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
