Findings
The facility failed to ensure supervision and staff support regarding known behaviors related to the Alleged Victim (AV) and Witness #1 (W1). An incident occurred between the two where W1 hit AV and AV fell off his/her scooter, causing a bruise. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
