Findings
Witness 1 (W1) has a known history of wandering into other resident rooms. On or about June 26, 2022, W1 knocked on the AV’s door and got into a resident-to-resident altercation. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
