Findings
The Alleged Victim (AV) has a history of leaving the facility unattended. On or about June 28, 2022, AV left the facility unsupervised and was found in a field about 1 mile from the facility. AV suffered 2 cuts on his/her legs and multiple abrasions. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
