The facility failed to intervene when AV’s condition changed resulting in repeated falls. The failure resulted in AV experiencing an unwitnessed fall, sustaining a right hip fracture, requiring further medical evaluations and admission to hospice, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
