Findings
On or about, June 29, 2022, Alleged Perpetrator 2 (AP2) did not follow proper procedure when showering the Alleged Victim (AV), which caused unreasonable discomfort and resulted in bruises on AV's hands/arms. AP2s actions are considered physical abuse. The facility failed to protect a resident from physical abuse which violates Oregon Administrative Rules.
