Findings
The facility failed to ensure supervision and staff support regarding known behaviors related to Alleged Victim (AV). An incident occurred where Witness #2 (W2) wandered into AV's room and the two hit each other. The facility failed to provide a safe environment for AV. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
