Findings
The facility failed to provide appropriate services according to Alleged Victim (AV)'s needs, relating to his/her number of falls with lack of interventions and AV's known history of not using the call system appropriately. AV experienced approximately fourteen unwitnessed falls from approximately May 18, 2022, and June 29, 2022, which resulted in repeated unreasonable discomfort as a result of some head injuries and skin tears. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
