Findings
The facility failed to appropriately care plan regarding the alleged victim's (AV) risk of falls to ensure safety. AV fell approximately four times within 90 days, with no new interventions put in place. Two of the falls in June of 2022 lead to multiple bone fractures, causing pain and putting AV at risk of serious harm. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
