Findings
The facility failed to implement interventions and appropriately care plan related to AV’s history of falls. On or about July 1, 2022, AV was found on the floor with a head injury and was transported to the hospital where he/she received stitches. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
