Findings
On or about July 2, 2022, Alleged Perpetrator #2 (AP2) was dispensing medications and gave the Alleged Victim (AV) the medications meant for another resident. AV suffered dizziness, pain and discomfort due to receiving medications that did not belong to him/her. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility failed to provide a safe medication administration system, which is a violation of Oregon Administrative Rules.
