Findings
On or about July 3, 2022, the Alleged Victim (AV) suffered a fall, resulting in a laceration to his/her head, after trying to get into a decorative baby stroller at the facility. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
