Findings
The facility failed to follow the AV's care plan to ensure s/he is wearing non-skid socks to bed to mitigate the risk and/or injury of falls. The failure resulted in AV experiencing an unwitnessed fall and was transferred to the hospital for stitches, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
