Findings
Alleged Victim (AV) is a high fall risk and is care planned for two-person assist. On or about July 10, 202, it was reported Witness 4 (W4) attempted to transfer AV alone and AV slid off the edge of the bed. AV did not sustain injury. The facility failed to follow AVs care plan, which is a violation of resident’s rights is neglect of care and constitutes abuse.
