Findings
On or about July 9, 2022, staff were dressing the Alleged Victim (AV) and found a bruise on AV's thigh and back of the leg. It is unknown how AV received the bruise. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
