Findings
On or about July 4, 2022, the Alleged Victim (AV) made an allegation of potential sexual contact with Alleged Perpetrator 2 (AP2) and on or about, July 13, 2022, made another allegation of potential sexual contact with AP2. The facility failed to ensure resident rights and safety when an internal investigation was not completed on or about July 4, 2022, which placed AV at risk of additional sexual contact. This failure is a violation of resident rights, is considered neglect of care and constitutes abuse. The allegation that AP2 sexually abused AV was investigated, and the findings were determined to be not substantiated.
