The Alleged Victim (AV) and Witness #1 (W1) engaged in an altercation where AV sustained a cut and a bruise. AV is care planned for altercations while W1 is care planned for wandering. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
