Findings
Alleged Victim (AV) is scheduled for showers twice a week in the evenings. AV had not been receiving showers by facility staff and so AV attempted to shower themselves and fell resulting in a hip fracture. The facility failed to provide service around showers to AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
