Findings
According to documentation Alleged Perpetrator (AP2) failed to protect the Alleged Victim (AV) from physical abuse. The failure resulted in AP2 forcibly closing AV’s mouth to prevent them from yelling, causing AV to wince in pain, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
