Findings
The facility failed to follow the AV’s care plan to ensure s/he was wearing non-skid socks to mitigate the risk of falls. The failure resulted in AV experiencing two unwitnessed falls in one night, causing unreasonable discomfort and bruising to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
