Findings
On or about July 19, 2022, Alleged Perpetrator #2 (AP2) was assisting the Alleged Victim (AV) with a brief change. AV was being combative and AP2 was holding AV’s forearms to restrain him/her. Other staff arrived and noticed AV had a skin tear on his/her arm and requested AP2 leave and allow other staff to assist AV. AP2's actions are a violation of resident rights, are considered neglect of care and constitute physical abuse. The facility failure is a violation of Oregon Administrative Rules.
