Findings
On or about July 23, 2022, the Alleged Victim (AV) suffered a fall, causing bruising to his/her left arm, pain to his/her leg and a cut to the back of AV's head. The facility failed to follow the care plan to have a fall mat by AV's bed. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
