Findings
The facility failed to follow the care plan for the Alleged Victim(AV) to reduce the risk of fall, by not applying grip tape to the floors during a temporary room relocation. This caused AV to fall and resulted in a skin injury, putting AV at risk of harm and unreasonable discomfort. The failures are a violation of resident rights, are considered neglect of care and constitute abuse.
