Findings
Alleged Victim (AV) has sustained prior falls. On or about July 22, 2022, AV was standing in their apartment doorway calling for help. AV stated he/she was getting up from their wheelchair and hit his/her arm causing a large skin tear with bruising on the inner aspect of the right forearm. The facility failed to provide a safe environment for AV, which is a violation of resident rights is neglect of care and constitutes abuse.
